Physical Stroke Recovery

Record your daily activities, mobility, self-care and symptoms. Use this checklist to notice changes, recognise small wins and prepare questions for your rehabilitation team.

Arm & Hand Rehabilitation

Card text: Keep track of arm and hand practice, everyday tasks and any pain or stiffness. Record what feels manageable and what support you need, alongside your agreed therapy plan.

Speech & Communication

Record your speech, language and everyday communication practice. Note difficulties, fatigue and small wins to help you prepare for conversations with your speech and language therapist.

Cognitive & Emotional Recovery

Keep a daily record of concentration, memory, mood and mental energy. Notice what helps, what feels difficult and which concerns you would like to discuss with your care team.

Vision & Eye Problems

Record how changes in your vision affect reading, movement and daily tasks. Note eye strain, visual fatigue and anything you would like to discuss at your next eye care appointment.

Eating & Swallowing

Keep a daily record of meals, drinks, comfort and swallowing concerns. Use it alongside your personal swallowing plan to note changes and prepare questions for your care team.

Bladder & Bowel Problems

Record bladder and bowel habits, comfort and practical routines without judgement. Notice patterns and changes so you can explain your concerns more clearly to your care team.

Sleep Disorders

Keep track of sleep times, night-time difficulties and how you feel during the day. Note what helps or disrupts your sleep, and prepare questions for your GP.

Medication & Stroke Prevention

Organise notes about your prescribed medicines, daily routines and follow-up appointments. Record missed doses, possible side effects and questions to discuss with your clinician or pharmacist.

Relationships & Caregiving

Reflect on communication, changing roles and the support you give or receive. Record what helps you connect, respect each other’s independence and make time for rest.

Returning to Work

Keep track of work tasks, agreed adjustments, breaks and energy levels as you plan or try a phased return. Record what works and what needs reviewing with your support team.

Driving After Stroke

Organise clinical advice, licence checks and practical questions about returning to driving. Use it to prepare for discussions with your care team; completing it does not confirm fitness to drive.

Young Stroke Survivors

Reflect on how stroke affects work, family life, relationships, identity and energy. Identify what matters most to you now, the support you need and one manageable next step.
